Basketball Experience
Medical Treatment Authorization: I authorize emergency medical treatment if necessary.
Assumption of Risk: I understand basketball involves inherent risks and release Hoops & Habits Limited from liability except in cases of proven negligence or intentional misconduct.
Photography & Video Consent: I permit photographs and videos of my child to be used for promotional, educational, website, social media and sponsor purposes.
Camp Outings Permission: I permit my child to attend supervised camp outings and transportation.
Behaviour Agreement: My child will follow camp rules and may be dismissed for serious misconduct.
